Vision Plans

Participants in our vision plans can choose from two providers: Davis Vision by MetLife and Vision Benefits of America (VBA). Each provider offers two plan options. The benefit providers and options differ based on:

  • coverage levels for various services and products
  • frequency of covered services
  • network of participating providers
  • process for obtaining services

Check both the and the to see which providers participate in each plan. You can also call your provider and ask if they participate in either of the plans.

Eligibility

You must be full-time benefits eligible (37.5 scheduled hours per week) to enroll. Members of the Campus Police Association must be scheduled to work 40 hours per week to be full-time benefits eligible.

Monthly, pre-tax rates are shown; divide rate by two to obtain biweekly, pre-tax rates.

Davis Vision and VBA offerings include a laser vision correction discount; blended, no-line bifocals (progressive lenses); and polycarbonate lenses (restrictions apply). See the Summary of Benefits for each plan.

How to Get Vision Care

Davis Vision by MetLife

  • During your appointment with a participating provider: show the provider your Davis Vision by MetLife member ID card. The provider will submit the claim to Davis Vision.
  • When using a non-participating provider: pay for the service in full, and then submit a Reimbursement Claim [pdf] at the out-of-network level.

Vision Benefits of America

  • Participating providers submit claims to VBA using participants' personal information. They will ask you for the last four digits of your Social Security number, date of birth and home zip code.
  • When using a non-participating provider: you will need to pay in full and then submit an with receipts.
